"Blank/White" Icon

Whenever I install a new program the shortcut on the desktop and the icon in the start menu is showing as a "white paper" or is "blank". The program itself works and starts fine, when I go to properties and change the icon I have to go thru the process of selecting the original shortcut icon (twice) before it applies the icon.

Any suggestions are welcome.
